The DANB RHS exam for dental x-ray duties

Facts read in DANB's Candidate Handbook and 33 other documents listed under Sources

The DANB RHS exam (Radiation Health and Safety) is DANB's national knowledge exam on dental radiography and radiation safety. It runs 75 questions in 60 minutes, all on digital imaging, and has no eligibility requirements. As of our November 2026 check, DANB lists its RHS exam application on the dental assisting pages of 24 states and the District of Columbia; each of those boards decides what a pass lets you do.

New Mexico, since April 21, 2026, accepts the RHS exam or a board-approved course for radiography certification, each valid for 24 months. Listed by DANB; whether RHS is required, one of several routes or optional is set by each state board (read Nov 6, 2026).DANB: State requirements for dental assistants [7][8]

A certificate, not a certification

A pass earns a certificate of knowledge-based competence in radiation health and safety, not a certification. The difference matters when an employer or a board asks what you hold: the certificate shows you passed one knowledge exam, while a certification (such as the DANB CDA) carries eligibility rules and yearly renewal.

The same exam is also a component of that certification (GC + RHS + ICE). A radiography pass counts toward it when the other two parts are passed within 5 years, so assistants who plan to certify later lose nothing by starting here.

What the certificate does not do is authorize x-rays on its own. Permission to expose images comes from the state dental board, which may name this exam, its own course or exam, or a combination; the state table further down shows what DANB lists for each state.

Who can sit it, and how long it lasts

Anyone can apply. There is no diploma, program, work-hour or CPR requirement, so students and assistants in their first weeks can take it before anything else. The other exam that starts without prerequisites is Infection Control (ICE); the General Chairside part of the certification is the one that asks for an education or work pathway.

There is nothing to renew: the certificate does not expire, and no continuing education is attached to it. A state can still put its own clock on a result; New Mexico is one example. There, radiography certification is earned by passing the RHS exam or a board-approved course with an exam, each valid for 24 months (in effect April 21, 2026).

Three domains, all digital

Three domains make up the outline effective March 12, 2025:

Every question assumes digital receptors. Darkroom chemistry and film processing are gone from the exam, so a guide built around them teaches content you will not be asked.

24 states and the District of Columbia; every row read Nov 6, 2026. “≈” = listed by DANB; whether it is required is set by the state board. A listing means DANB offers its RHS application on that state's dental assisting page, as of Nov 6, 2026.

California and Texas are missing from this table on purpose: DANB's pages for both list the DANB CDA application but not this exam, and both states run their own radiography training.

Three listed states add a radiography credential of their own alongside the national exam or in its place:

  • DANB also lists Maryland's Dental Radiation Technologist requirements.
  • DANB also lists a combined New York Professional Dental Assisting, RHS and ICE exam (NYPDA/RHS/ICE).
  • DANB also lists the Oregon Radiologic Proficiency (ORCR) certificate, Pathways I and II.

Ohio is not in the table. DANB lists no RHS exam application for Ohio; it lists its RHS Review course and an Affidavit of Ohio Clinical Radiography Training form.

Rules on who may take x-rays also shift with supervision law, as in Connecticut. A dentist who delegates dental x-rays to a dental assistant takes responsibility for them but need not stay on site (in effect October 1, 2026). Check the board before you rely on any row here.

When mounting bitewings, which feature helps confirm correct orientation?

Pick the option you would mark on test day. The choice stays fixed, and every option then explains itself.

  1. Larger crowns placed on the viewer's left
Crown size says nothing about the side of the mouth; using it to orient an image is a guess that flips left and right.
  2. Occlusal plane curving upward toward the molars
The upward curve of the occlusal plane toward the molars (the curve of Spee, a smile shape) confirms the maxillary teeth are on top and the image is not flipped.
  3. Premolars placed toward the outer edge of the mount
Premolars belong toward the center of the mount because they sit closer to the midline; putting them at the outer edge reverses the image.
  4. Maxillary crowns shown in the lower row of the image
Maxillary teeth are displayed in the upper row; showing them below means the image is upside down.

The reduction in x-ray beam intensity as the beam passes through the patient's tissues is called:

  1. Ionization
Ionization is what happens to atoms when photons interact with them; it is a result of the interactions, not the name for the drop in beam intensity.
  2. Thermionic emission
Thermionic emission is the release of electrons from the heated cathode filament inside the tube, before any x-rays exist.
  3. Attenuation
Attenuation is the loss of beam intensity as photons are absorbed or scattered by the tissues they pass through.
  4. Rectification
Rectification is the conversion of alternating to direct current in the x-ray circuit; it has nothing to do with tissue.

When should the sterilization pouch that holds a receptor holder be opened?

  1. At chairside, right before use
At chairside, right before use, the pouch is opened in front of the patient, so the holder stays sterile until the moment it goes into the mouth.
  2. In the sterilization area, right after the cycle ends
Opening it in the sterilization area right after the cycle ends exposes the holder to air and handling on the way to the room, so sterility is lost early.
  3. After the procedure, during room cleanup
Opening the pouch after the procedure, during cleanup, means the holder sat unused or was used without its sterile wrap; neither protects the patient.
  4. At the start of the day, for every patient on the schedule
Unwrapping holders for the whole day in advance leaves them sitting exposed for hours, which defeats the purpose of packaging.

Questions about the RHS certificate

Does passing the RHS exam let me take x-rays in any state?

No. The exam is national, but x-ray duties are licensed by each state board. Some boards name this exam, some use their own course or exam, and some combine routes. Start with the state table and confirm with the board.

Is the RHS certificate the same thing as the DANB CDA?

No. It is one of the three exams behind that certification. Passing it alone gives you a certificate; adding General Chairside and Infection Control within the time limit gives you the certification. See the DANB CDA overview.

Do I have to finish a dental assisting program before applying?

No. Anyone can apply for the RHS exam; program and work-hour rules apply to the General Chairside exam, not to this one.

Does the radiography certificate ever need renewing?

Not with DANB: it does not expire and has no CE requirement. A state may limit how long a result counts toward its own credential, so check the board if your pass is several years old.
